Home Health Care is an incredibly wonderful service supplied to patients in their homes in our community.

We are hiring a Full Time Administrative Assistant

Summary

This position facilitates the intake/admission processes for all Skilled Services for the Home Health Care program,. Includes providing information, consultation, community liaison, and assistance for all inquiries regarding home health care services. Assist with matching client needs with agency services and reimbursement sources; and coordinating with home health care staff for initial client assessment and planning. Assist with continuity of care and discharge planning in hospitals. Promotes goals of achieving patient outcomes thought appropriate screening and maximizing reimbursement thought concurrent review.

Essential Duties and Responsibilities

These position responsibilities will vary from intake and client care process, to community liaison.

Coordinates and facilitates potential clients for the appropriate home health program for all referrals.

Assumes initial responsibility for coordinating with the Director of Clinical Services for assessing age/diversity specific needs/issues of patient/ family being served; and consulting with physicians, discharge planners and other home health staff members.

Facilitates discharge planners with appropriate use of home care services to meet their discharge plan.

Determines patient’s appropriateness for home care services consistent with 1st Choice Home Health polices and admission criteria and in conjunction with the clinical supervisor and patient’s physician.

Make telephone contacts and/or assessment visits to hospitals and/or long-term care facilities for potential home health/hospice clients to provide information on 1st Choice Home Health programs, and extended programs, of care.

Provides information and consultations to clients, potential clients, families, significant others, physicians, hospital staff, care coordinators, and to the community in general to assess home health care needs, and make appropriate referrals to other community resources.

Utilizes sound judgment and applies knowledge of admission process, systems, and skills in the performance of duties. Interfaces with external review companies, using persuasion and negotiating skills, regarding specific clients, for medical necessity and appropriateness of home health care.

Works cooperatively and in a positive manner with community and interdisciplinary members to maximize the efficient and appropriate delivery of home health care services.

Participates in agency’s orientation, in-services, weekly case conferences, monthly team meetings, training programs for professional staff, and utilization review.

Documents, communicates, and files accurate and timely information appropriately.

Assists all team members with Oasis questions, processes, and duties.

Performs other related duties as requested by the Director of Clinical Services.

Assists the nursing staff with scheduling and rescheduling patient appointments.

Daily monitors the HomeSolutions Validator and other reports as required, and takes action to resolve all conflicts on reports on the Electronic Health Record for 1st Choice.

Run supply errands and other activities as directed.

Qualifications:

Maintain a current Alaska’s Driver’s license.

Working knowledge of criteria for reimbursement from Medicare, Medicaid, commercial insurance, self-insured plans, and other funding sources.

Knowledge of payer requirement for admission and discharge criteria for home care patients.

Must have the ability to communicate well and interact with multiple health care professionals, including medical staff.

Organizational and problem solving skills as well as ability to work independently required.

Regular attendance is a requirement of this position.

Must be able to read, write, and speak English.

Must have strong attention for details!

Medical Experience preferred but not necessary.
