Must have experience working directly with clients or consumers.

Independent Living Advocate-Older Blind, DD, Pre-ETS Job description Summary/Objective

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Essential Functions:

Goal Development

Assesses the consumers long- and short-term needs and goals during the initial contact.

Establishes eligibility or makes appropriate timely referrals with consistent follow-up.

Structures their Independent Living Plan, assures that appropriate documentation is signed,

and establishes individual goals in a timely manner.

Formulates and updates Habilitation plans for every OB consumer, including a brief history of

presenting problems and goals for services, and develops proper Core plans for eligible

consumers monitor expenditures and completes purchase orders.

Skills Training

Structures and provides consumer training based on individual goals or service requests.

Schedules or facilitates appropriate training, concentrating on home and community-based Independent

Living

skill development and individual responsibility.

Helps consumers access resources and information including multi-agency collaboration and

education regarding their rights, how to self-advocate for their needs, and how to complete

the administrative process and paperwork needed to secure those services.

Provides vocational and employment skill building activities (i.e. resume writing, interview

skills, professionalism, job search skills) to assist consumers to find and sustain consistent

work.

Service Coordination

Assists in developing solutions to disability and daily living problems as well as systems problem solving in a

timely and consistent manner.

Assists with locating peers, professional and role models in the community.

Contacts agencies and service providers to ensure integrated service delivery, multi-agency

collaboration using professional and effective communication and outreach skills evidenced by timely results.

Attends community events and meetings related to IL issues and services as assigned by

supervisory staff.

Be familiar with all aspects of Older Blind services including: Core Plans, Older Blind grants and resources

in the community.

Coordinates daily and weekly schedule with consumers and other staff; routinely reviews

service plans with Supervisor.

Helps provide consumers with access to Assertive Technology, Computer lab and additional Access Alaska

equipment.

Enters goals, all contacts and demographic information in CIL Suite or telephone logs in a timely, thorough,

accurate, and consistent manner.

Available and responsive to assist walk-in consumers.

Advocacy

Participates in efforts that empower people with disabilities to create lasting systems

changes.

Conducts meetings/workshops to heighten awareness & respect for personal disabilities.

Provides testimony and letters of support for individual consumers.

Networks with community resources to ensure an understanding of Access Alaskas services and the services

available in the community for our consumers.

Assist consumers with personal advocacy tasks.

Other duties as assigned.

Work Environment:

This job operates in a professional work environment and on site supported employment locations in Fairbanks and surrounding areas.

Physical Demands:

While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; and taste or smell. The employee must occasionally lift or move up to 20 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.

Equipment Used:

This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Non-Exempt/Hourly/Overtime Eligible

Work Hours:

This is a full-time position. Days and hours of work are typically Monday through Friday, 8:00 a.m. to 5 p.m. including a 1-hour non-paid lunch. There will be times the set hours will be changed to meet the demands of Pre ETS, of Consumer need.

Travel:

Pre ETS requires transporting of students with in Fairbanks and the surrounding area during the summer and start of the school year using the Access Alaska passenger van.

Required Education Experience:

Four (4) years of education Or a combination of education and experience equaling 4 years, in Health & Human Services. Prefer experience in the Independent Living and Consumer Directed Services. Mobility Specialist Certification preferred or the ability to obtain certification within 1 year of employment. Must have experience working directly with clients or consumers.

Working Authorization:

All positions at Access Alaska Inc. require a State of Alaska fingerprint background check prior to starting work; that results in receiving first a Provisional approval and finally a Five (5) yr. Approval.
