Position Available: ACT/MOCR Nurse (Assertive Community Treatment/Mobile Outreach Crisis Response)

Description: The ACT/MOCR Nurse, under the direction of the ACT/MOCR Team Leader, will function as a member of the ACT team in determining the most appropriate treatment regimen in meeting the identified needs of individuals with severe/persistent mental illness and co-occurring substance use disorders with the majority of services completed in the community. Responsible for medication coordination with pharmacy and physician (medical and psychiatric), interact and educate both staff and recipients about psychotropic medications, work closely with Prescribers regarding medication training/administration. Provide nursing assessments to include monitoring of recipient’s health care needs. This position will also provide on-site or mobile assessments and referrals for further treatment to any individual experiencing a mental health or substance abuse problem, or both. Services are provided to children, adolescents, and adults in various community settings, homes, and at the emergency department within the catchment area. Our staff assess clients for critical symptoms and work with the client to coordinate their next phase of treatment.

SeaView Community Services is the premier provider of Behavioral Health services for the Eastern Kenai Peninsula, serving as the Community Behavioral Health Clinic and offering key programs in both Mental Health and Substance Use. SeaView works diligently every day to expand access and utilization of behavioral health services with the goal of improving the mental health status of our community and decreasing substance misuse. We are excited about the opportunity to add a new professional Nurse to our team to support us in accomplishing great things for our community.

Essential Functions:

ACT Team

1. Assist prescriber and all ACT service recipients with ongoing medication management across a variety of community-based settings.

2. Administer IM and PO medications in a variety of community settings as directed by Prescriber.

3. Provide medical screening, treatment, referral and support. Accompany ACT service recipients to medical appointments when indicated, coordinating health care services with SeaView as well as all healthcare providers throughout the community as needed.

4. Assist clients in completing necessary lab work/prepare and send out labs.

5. Provide client education and support regarding medications and medical concerns.

6. Coordinate medication management including pick-up and delivery through pharmacy to individuals receiving ACT services throughout a variety of community locations.

7. Triage medical needs.

8. Provide appropriate referrals for medical and dental needs and assist as needed.

9. Maintain all necessary documentation and clinical records.

10. Review current medication management protocols and assist in updating as needed.

11. Provide in-service training for ACT staff on common medical issues and appropriate treatment, intervention, and support strategies.

Mobile Outreach Crisis Response

1. Functions effectively as a member of the Mobile Crisis Team.

2. Provides timely and effective evaluations of client’s behavioral health and somatic concerns.

3. Demonstrates knowledge and ability to provide a full range of nursing care to clients with a variety of withdrawal symptoms, other somatic issues and behavioral health problems.

4. Provides medication education for clients, including uses and potential side effects.

5. Administers medications, monitors clients for potential side effects of medications and report findings to the physician.

6. Order medication refills as necessary and make arrangements for medication refills to be picked up.

7. Maintain accurate and complete documentation including review of MARs for accuracy, ensuring MARs updated as needed due to additions/deletions of medications, changes in dosage, etc.

Location: Seward, AK

Skills: Must have excellent communication and organizational skills. Ability to simplify concepts and teach others. Ability to work with others to ensure records compliance. Ability to establish and maintain effective working relationships with staff and clients.

Licensure/Certification: Licensure by Alaska Board of Nursing required (LPN or RN, pay commensurate with credential); valid driver’s license, satisfactory driving record and CPR/First Aid Certification.

Other: TB test, pre-employment drug/alcohol screening, criminal background check.

$30-$37/hour DOE and licensure.
