Title: Direct Care Provider (DCP)
Reports to: Program Administrator
FLSA Status: Non-exempt
Wage Contract: $14.00 - $15.00/hr DOE
Employment type: Part-time/Full-time
Hours of operation 8a - 8p
General Overview: Caregivers will be responsible for implementing the client's Plan of Care (POC) and will be directly responsible to the Program Director.
Job Duties:
Accompany clients on individual and group activities in the community while engaging them in the goals
identified on their POC
Attend initial and ongoing training/meetings as requested by Program Administrator
Pick up, transport and return client to identified location in personal vehicle
Implement/participate in the activity schedule as identified by Program Administrator
Provide/assist client with personal care needs as necessary
Assure that health and safety needs are met while caring for clients
Document recipient services and other events as assigned and on time
Light housekeeping of the facility
Keep accurate record of mileage for reimbursement and submit every Monday
Other job duties as assigned
Qualifications (required):
18 years of age
High School Diploma or GED
No felony convictions and able to pass criminal background check
Current Driver's License with acceptable driving record as determined by our insurance requirements
Safe and dependable vehicle with proof of insurance to transport clients
Current CPR/First Aid certification
Annual PPD test
Qualifications (recommended):
Six months of experience or training providing direct care to human service recipients
Hepatitis B immunization (or) waiver
Knowledge, Skills and Abilities
Ability to communicate effectively, both verbally and written
Demonstrate proficiency in direct care activities
Ability to engage clients and adapt activities to client's abilities
Ability to work independently, learn intervention strategies and prioritize activities
Reliable, respectful, enthusiastic, punctual and professional
Physical Requirements:
Stooping- Bending body downward and forward by bending spine at the waist.
Crouching- Bending the body downward and forward by bending leg and spine.
Reaching- Extending hand(s) and arm(s) in any direction.
Standing- Particularly for sustained periods of time.
Walking- Moving about on foot to accomplish tasks, particularly for long distances, up to, but not limited to
two miles.
Pushing- Using upper extremities to exert force in order to draw, press against something with steady force in
order to thrust forwards, downward or outward.
Pulling- Using upper extremities to exert force in order to draw, drag, haul or tug objects in a sustained
motion.
Lifting- Raising objects from a lower to a higher position or moving objects horizontally from position-to-
position. Ability to lift 25lbs on regular occasion
Additional Contract Items (DCP): MSR may provide Office Support (OS) hours as required to accomplish these duties during the week. These hours are intended for use by DCPs specific duties only. MSR is an "at-will" employer. This means employment can be terminated at any time for any reason permitted by law.
