ESSENTIAL DUTIES AND RESPONSIBILITIES

Develop the person-centered plan with the individuals being served, natural supports, professional supports, and other team members.

Provide input into person-centered plan modifications, as needed.

Coordinate activities within the established plan and facilitate meetings.

Assist with referral linkage to other services and community resources.

Provide direct intervention, arrange, coordinate, and monitor services on behalf of the person’s supported.

Minimize the negative effects of psychiatric symptoms or substance dependence that interfere with the person supported daily living and personal development.

Support the development of various independent living skills and clinical interventions specific to the person’s goals and mental health diagnosis.

Complete documentation of services including the name of the person supported, Medicaid identification number, record number, date of service, purpose of contact, task/interventions performed, the time spent performing the interventions, effectiveness of the intervention, the signature and credentials of the staff providing the service.

Supervise paraprofessionals, Associate Professionals, and Certified Peer Support Professionals to include developing a supervision plan and documenting supervision meetings.

Work closely with other Associate/Para-professional/Certified Peer Support Professionals and clinical/professional staff to maintain communication and provide feedback, standardize. procedures within the agency.

Facilitate relationships and serve as a link between the agency, parents, guardians, natural supports, other service providers, and the community.

Facilitate and/or participate in treatment team meetings (if indicated).

Serve as first responder for agency clients.

Ensure that all documentation is accurate and reflective of services and completed on time as defined by the agency’s policy and state guidelines.

Ensure authorizations remain current and meet appropriate medical necessity for the services being provided.

Assist with the development and design of training modules for staff within the agency and administer staff trainings, as needed.

Other duties as assigned.

EDUCATION AND EXPERIENCE

BS/BA degree in social work, psychology or related field with four years’ experience in mental health, substance abuse recovery or case management OR Master’s degree in social work, psychology or related field with one-year experience in mental health, substance abuse recovery or case management.

Knowledge of group dynamics and commitment to client-centered work; experience with groups including planning, teaching, and co-leadership responsibilities.

Strong written and oral communication skills, and analytical and decision-making skills.

Proficient in Microsoft Word and Excel.

Must possess a valid state ID or driver’s license and pass a criminal background check.

Ensure confidentiality regarding sensitive material and individual rights to privacy, and protected health information for the persons supported.
