The Statewide New Services Coordinator (SNSC) provides guidance and oversight throughout the New Services and Admissions process to ensure individuals who qualify for developmental disabilities (DD) services receive needed supports. The SNSC introduces individuals, families, and Conflict Free Case Managers (CFCM) to Hope Services and systematically guides individuals, families and their team through the Hope intake and planning process. This position supervises assigned staff and ensures all necessary and current documentation is gathered in order to assess and identify the needs and strengths of individuals for the purpose of ensuring appropriate services are inplace. The SNSC facilitates and provides follow up with all aspects of New Services, Executive Review (Action Team) and Admissions processes. The SNSC provides Hope Planning Teams with direction, oversight and decision-making as needed that comes out of the Executive Review team. The SNSC is responsible for all aspects of outreach and education related to New Services.

The SNSC applies Hope’s mission, vision, beliefs, values, and expectations into every aspect of operations while working with individuals, families and their teams to ensure fair and equal access to all services and supports. The SNSC is an independent advocate for the individual.

Job Duties:

1. Guides all regions through New Waiver, external transfers, Intra-Agency transfers, intensive need cases and discharges from the Agency.

2. Supervises, develops, evaluates, counsels, and disciplines staff through teaching, coaching and mentoring. Collaborates in all aspects of onboarding personnel to fulfill department requirements. Ensures a quality team approach to New Services, Executive Review (Action Team) and Admissions processes.

3. Serves as primary Hope contact for all Agency inquiries related to DD services. Ensures proper parties become involved, and provides guidance and oversight as needed. Fosters and maintains excellent working relationships with all Alaskan agencies.

4. Initiates meetings with individuals, families and Conflict Free Case Managers (CFCM) to conduct person-centered planning. Mentors team members across the state in developing skills for conducting purposeful and concise meetings, while continuing to foster meaningful relationships between Hope teams and individuals, families/guardians, CFCMs, etc.

5. Trains statewide on Executive Review (Action Team) processes, and facilitates these teammeetings to assist in access to supports as driven by the individual/family.

6. Develops and distributes intake and planning information to all Hope Directors in a timelymanner.

7. Creates statewide new service operations and procedures to ensure statewide replication and quality end product.

8. Ensures all required internal and external paperwork is completed correctly and timelyto secure waiver services and determine support needs.

9. Develops, implements, and oversees Hope New Service Orientations for individuals and families seeking Hope’s services and supports.

Job Requirements:

Bachelors’ Degree in Health Science, Social Science, Education, or closely related human services field required. Experience in lieu of education may be accepted on aone for one basis in appropriate fields.

Master’s degree preferred.

Eight years of experience working with families, state services and service providers required, involving responsibility for planning, development, management, or operation of programs and service delivery, needs assessment, program evaluation, or similar tasks.

OR

An equivalent combination of related education and/or experience.

Addt'l requirements:

Possesses a valid Alaska driver’s license and has at least one year of driving experience (may include permit time).

Able to safely operate Agency vehicles when needed. Possesses reliable transportation and vehicle insurance when transporting support recipients or conducting Agency business.

Meets State and Agency requirements for passing a criminal background check upon hire and as required thereafter.

Lifts 25 pounds from a squat and kneel position on the floor.

Works at desk/computer workstation for a minimum of six hours per day.

Completes all required State, Agency, department, and position specific training as assigned.

Obtains a tuberculin clearance at time of employment and annually thereafter.
