Optum Complex Care Management provides the Transitions Model of Care for United Healthcare members in a short stay/transitional setting with focus on reduction in 30-day hospital readmissions, reduced ER visits, optimized SNF length of stay, improved completeness and coding accuracy of diagnosis and medical record documentation, increased closure of STAR/HEDIS quality measures, improved completion rates of Advanced Care Planning Directives, and improved patient and family satisfaction. The Optum practitioner will follow the member until the member is discharged from the TTS program and will conduct a focused visit within 24 business hours of notification, provide a comprehensive visit within 7 days, conduct telephonic or in-person care coordination clinical rounds days 2 through 14 and then every 5 days thereafter. This is a flexible, autonomous role that creates enormous satisfaction for the NP as you impact the care and comfort of our aging population. Provide preventative and primary care for patients in assigned long term care facilities and SNF's

Work with primary care physicians to provide the best care possible

Collaborate with the nursing staff and the patients' families

Work with the facility's utilization review process

You’ll

Current, unrestricted NP license in the state of assignment or the ability to obtain

Ability to obtain DEA licensure/Prescriptive Authority post-hire in states where applicable

Nurse Practitioner national certification as ANP, FNP, or GNP required or the ability to obtain

A clinical background in adult, family or geriatric specialties

Past experience working in a nursing home or with seniors in other settings

Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook

1+ year, hands on post grad experience within Long Term Care

Weekday and weekend availability

Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area

Ifyou need to enter a work site for any reason, you will be required to screenfor symptoms using the ProtectWell mobile app, Interactive Voice Response(i.e., entering your symptoms via phone system) or a similar UnitedHealthGroup-approved symptom screener.
