Brookside Senior Living Community is looking for a caring and compassionate RN or LPN to be full-time or part-time nurse. This responsibility includes the management of staff, services, documentation, resident care, quality assurance, and regulatory compliance. Our facility is a 16 bed / 16 resident facility that is family owned and operated and has had a complete remodel and renovation recently.

He or she will ensure that the policies and procedures established by Brookside Management are implemented and maintained consistently. You report directly to the Administrator/DON and have an open dialogue with Brookside ownership.

Relationships:

Supervision of staff and coordination with individuals includes the following:

Key management staff, department leaders and direct care staff

Consultants and/or Vendors

Physicians, Nurses and Health Care Providers

Residents, family members, support groups and referral sources

Federal, County, State and City officials

Responsibilities and Duties Areas of Responsibility :

Planning

Assists in the development and implementation of policies and procedures as they relate to the residents health care.

Practices excellent time management skills and completes tasks according to physician orders and within company established time

Ensures effective communication occurs between wellness employees assigned to different shifts.

Maintains state and federal regulations and actively participates during regulatory surveys and monitoring medications according to physician' s orders and residents activities of daily living (hygiene/grooming; incontinence; supervisory); assessments and evaluations; therapies; reorder of medications; med cart reviews.

Organizational:

Delegates authority and tasks to qualified staff according to state law and

Develops staffing schedules to ensure that residents' needs are met and that staffing levels remain above the state

Develops and implements systems for monitoring resident's health status and treatments, including but not limited to labs, medication administration, coordination of health care services, medical appointments, staff response times to resident

Ensures assessments are performed according to company policy and procedures, including assessments related to a change of condition and/or an incident

Maintains an efficient and organized filing

Responds to state requests for information within the state required time frames. Immediately notifies administrator regarding any federal, state or local agency request(s) for

Notifies DON/Administrator promptly when a state or federal inspection or survey is in progress.

Hires qualified staff, provides training, supervises, evaluates and ensures fair treatment to all

Directs, supervises, documents and enforces resident care according to physician's orders.

Utilizes progress notes effectively to show coordination of

Implements a quality assurance program designed to improve resident care and documentation. Utilizes the community alert call software system daily to monitor employee response to resident

Performs comprehensive initial assessments and takes the lead role during the admission process as it relates to resident care, safety and well-b Ensures level of care is according to actual delivery of service and resident needs.

Recommends to the Administrator/DON matters and issues requiring policy decisions and/or

Leadership

Assumes overall leadership and administrative responsibilities for the wellness department

Delegate responsibilities in accordance with short-term and long-term

Develops positive working relationships with staff, residents, resident representatives, health care providers and Conducts family meetings related to resident care and well- being.
