Performs nursing care to patient in accordance with an established plan of care in accordance with Right at Home policies and procedures and all other laws and relevant professional nursing standards.

Essential Functions

Furnishes services in accordance with RAH policies and the Alabama State Nurse Practice Act, Administrative Code and Code of Conduct as applicable.

Prepares and records clinical notes and progress notes.

Observes and reports any signs or symptoms indicative of changes in conditions of the patient and family situation to the Nursing Manager/Director of Nursing and documents such reports and observations in the clinical record.

Performs assigned nursing procedures, including administration of treatments and medications, in the care of the patient, under the supervision of a Registered Nurse, in accordance with the Alabama State Nurse Practice Act and the Plan of Care.

Prepares equipment and materials for treatments observing aseptic technique as required.

Assists the patient in learning appropriate self-care techniques.

Assists with the rehabilitation of patients according to the patient care plan and/or MD orders.

Consults the Nursing Manager/Director of Nursing in making judgments or decisions regarding care and treatments.

Additional (non-essential) Functions

Participates in committees.

Participates in community relations activities.

Education, Experience, Knowledge, Skills, Abilities, and Availability

Graduate of an accredited school of Practical/Vocational Nursing Program and one year of nursing experience in the past five years or completion of an accredited refresher course within the past two years or a minimum of one year of recent experience in home health/hospice agency.

Current Alabama State license and in good standing as a Practical/Vocational Nurse in Alabama State Board of Nursing.

Knowledge and ability to provide skill nursing care defined in the scope of practice in the Alabama Board of Nursing Nurse Practice Act.

Passing score of 80% on the RAH Licensed Practical/Vocational Competency Examination.

Current CPR (BLS) Certification must be on-file to work as Nurse.

A valid driver's license and access to adequate transportation and be an insured automobile driver.

Available for work on weekends and holidays as requested.

Understanding of the working team concept and the ability to work under supervision and in collaboration with other care staff.

Meets the requirements for LPN providing direct patient care in Alabama.

Must be able to read, write, speak, and understand English as needed in the job.

Annual tuberculin skin test or chest x-ray (every 3 years to include a health statement signed by a licensed health examiner indicating TB free) and any other health tests or immunizations required by RAH or facility.

Drug screening testing upon hiring and random drug screening testing as needed/required. (I.e. immediate drug screen testing when there has been an on-the-job-injury, or motor vehicle accident regardless of who is at fault.) Refusal to obtain drug screen test will be disciplined and/or up to immediate termination.

In-Service Training

Assumes responsibility for personal professional development. Participates in in-service education programs as Alabama State law mandates.

Working Conditions/Environment

Primarily works in homes, however may also work in facilities, and requiring travel. Shift times and days vary with home and facility needs. This requires Nurse to work independently, autonomously and collaboratively with RN direction and other care staff, client and client's families. Problem solving skills are a must for this position and must take direction from Registered Nurse.

Notes/Special Instructions

May be called upon to perform additional tasks as assigned by Nurse Manager/Director of Nursing within the scope of the Alabama Sate Nurse Practice Act.
